Policies

  • After booking an appointment, all new clients will receive a brief phone call from their therapist to introduce themselves, discuss with you any concerns, questions, and goals for treatment, and to make sure we are a good fit.

    Prior to your first appointment, you will be required to complete an online confidential intake form (included in your welcome email). Please submit this at least 24 hours before your session so I can better prepare for your appointment.

  • Please make any changes to your appointment 24 hours in advance.

    If less than 24 hours’ notice is given, the cancellation fee is 50% of the full session rate.

    If 2 hours’ notice or less is given or completely missed the cancellation fee is 100% of the full session rate.

  • We require a credit card on file or prepayment in order to reserve your appointment. Having a card on file protects our time in case you miss your appointment. It also gives you the option for contactless payment at your session. Your card WILL NOT be charged unless you violate the Cancellation/Missed Appointment policy. If you do not wish to save a credit card on file, please give us a call so you can prepay for your session.

  • If you have a contagious infection or illness, we will not treat you with massage until you are well. If you have a common cold, influenza, stomach flu, coronavirus, meningitis, shingles, contagious skin conditions, etc., you must not come into the office. Please call us before your scheduled appointment to cancel. The Cancellation Policy applies and is at our discretion. You can reschedule once your condition has resolved.

  • Time devoted for a thorough intake and post session recommendations are really important for me to evaluate your treatment plan and deeply listen to you during every appointment. For this reason, I build 25-30 minutes into the appointment time and want to make sure you know to plan accordingly.

    Scheduling options will continue to be categorized by “time on the table” (ie 30, 60, or 90 min) but please note the total time that is scheduled for each session.

  • In order to receive the best massage therapy possible, it is important to communicate ANYTHING and everything, including needs, preferences, requests, or feedback, at any time before, during, or after the massage.

    If there is anything distracting you, or you feel unwell or uncomfortable at any time during the session, please communicate so as soon as possible so adjustments can be made.

    I always want your HONEST feedback - positive or negative - and I don’t take offense to it.

  • You agree to be responsible for all service charges and fees. We accept cash, check, credit card or prepaid gift card at each session. Failure to provide timely and complete payment may prevent you from scheduling future treatment.

    I can accept HSA or FSA debit cards but recommend you confirm coverage with your carrier before using your card.

    I DO NOT submit insurance claims, I can provide you with an invoice upon request that you can submit to your insurance carrier for reimbursement.

  • I provide therapeutic massage services. You agree to let me know about any known medical conditions you have that might be affected by massage services. It is your responsibility to disclose any medical conditions to me before the start of your massage. Medical conditions include but are not limited to: Heart conditions, stroke, diabetes, skin conditions and cancers.

    I cannot diagnose any medical conditions or provide referral for medical treatment or tests

  • Only professional massage and bodywork services for relaxation or therapeutic purposes are offered at this massage practice, and are performed by licensed and insured massage therapists. I reserve the right to refuse or discontinue service at any time. Reasons may include, but are not limited to, the following:

    - Inappropriate / offensive, derogatory, or threatening speech or behavior

    - Sexual advances or requests

    - Intoxication or drug use

    - Physical or mental conditions for which massage is contraindicated

    - Contagious or unidentified skin conditions

    - Repeated no shows and/or late cancellations

    If you exhibit any of the above, you may, at therapist's discretion, be denied treatment and charged for the appointment. If a massage is terminated, no refund will be paid to the client. Louisville Therapeutic Massage retains the right to arbitrate disputes and determination shall be final.
